Help Us Document Our Performance On Year 2 Objectives

Thank you for participating in the SVTC Ed Tech grant activities and for your willingness to report your use of Ed Tech grant-related hardware, lesson plans, video streaming and Explore Learning.

Continued Ed Tech grant funding is contingent upon the SVTC meeting goals proposed in the grant. Some of these goals are dependent upon teacher usage. The purpose of this report form is to gather necessary data from teachers to document achievement of the goals.

This report consists of two parts: 1) Technology Readiness (Essential Conditions); and 2) Use of technology to support teaching and learning. Completing this report will take approximately 2 minutes for the Technology Readiness component and 1-2 minutes for each techology-enhanced lesson you report. Thank you in advance for taking the time to document progress toward our goals.

When you begin the survey, you will be asked to provide your division and school. Please note that no personally identifiable information is collected for this report.
